Will your carers actually use it?
That is the question that decides whether any of the rest of this is true. A carer at a door in the rain has about two taps of patience, and everything they need to know has to be on the screen before they knock.

At the door
The phone does not get in the carer’s way.
Check in is two taps: arm, then confirm. It opens fifteen minutes early and stays open for an hour after. It records GPS and the device clock as evidence the visit happened.
If a carer refuses location, they still check in. The record says so, and the visit goes ahead. A carer standing on a doorstep is never blocked by a permission dialogue — and you get an honest record instead of a missing one.
Before they knock, the visit screen shows them the access note and key-safe code, the risk alerts, what they must do, what they must avoid, when to escalate, and the handover from the last visit. The note afterwards is a guided form — eleven sections, with medication as a fixed set of answers rather than a blank box, and a “flag a concern” control inside the note itself.

Concerns
A concern raised at 11:42 is in your queue at 11:42.
When a carer flags something inside a visit note, it does not sit in a form waiting to be typed up at the end of the week. It arrives.
While it is open it shows on your evidence page as a Regulation 13 gap, so it cannot quietly become the thing everyone assumed somebody else had closed. If it is high or critical, HeraIQ raises your Regulation 20 duty of candour rather than leaving you to remember which threshold applies.
